The Parent PLUS Loan Disbursement Authorization Statement is required to disburse PLUS Loans. You can find the form below and fax it to (850) 644-5142 or mail it to our office:
Available funds will be disbursed according to the selection made on the Parent PLUS Loan Disbursement Authorization Statement (PAS) on the published disbursement dates. Parent Authorization Statements must be completed by the parent and one of the following selections must be made:
1) mail a check to the parent,
2) mail a check to the student,
3) EFT funds to the student's FSUCard account.
If your parent is receiving a PLUS Loan, you may be eligible for a tuition deferment if the loan has not arrived by the deadline. If you are not eligible for the deferment, you must pay tuition in full by the tuition payment deadline or you will be assessed the $100.00 late payment fee.
